29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees.     At a work place located at 311 Nichols St., Fort Worth, Texas: an employee was not provided a safe means of egress from approximately 13-foot deep excavation while checking on sewage pipes and was exposed to cave-in and crush-by hazards.

At a work place located at 311 Nichols St., Fort Worth, Texas: the employer did not ensure the operator digging the trench place the loose soil 2 feet away from the edge of the trench exposing employees to the hazards of cave-in.

At a trench on 311 Nichols St, Fort Worth, Texas:  an employee was exposed to cave-in hazards while checking on a sewer line inside a 13 feet depth trench with vertical walls on all sides and no protective system.
