29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 ppm: a) On or about March 29, 2018, an employee working in the seed building was overexposed to carbon monoxide at an 8 hour time weighted average of 336.3 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 672.6%. b) On or about March 29, 2018, an employee working in the seed building was overexposed to carbon monoxide at an 8 hour time weighted average of 543.50 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 1087.0%.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): a) On or about March 29, 2018, the employer failed to implement adequate engineering and/or administrative controls to reduce employee's exposure to carbon monoxide. An employee working in the seed building was overexposed to carbon monoxide at an 8 hour time weighted average of 336.3 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 672.6%. b) On or about March 29, 2018, the employer failed to implement adequate engineering and/or administrative controls to reduce employee's exposure to carbon monoxide. An employee working in the seed building was overexposed to carbon monoxide at an 8 hour time weighted average of 543.5 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 1087.0%.

29 CFR 1910.1200(h)(3)(iii):The measures employees can take to protect themselves from these hazards, including specific procedures the employer has implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used; and,: a) On or about March 29, 2018 the employer failed to train employees on the elements of a written hazard communication program including the hazards of the chemicals in their work area. Employees were exposed to hazards from carbon monoxide gas from the exhaust of a propane powered forklift used in the facility.
