29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a) On or about April 4, 2018, employees were exposed to falls of 15.5 feet from Sky Jack SJIII 3219 scissors lifts in that the workers stood on the guardrails of the lifts while welding bar joists and bridging, thereby defeating the fall protection attributes of the guardrails. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: (a) On or about April 4, 2018, employees were exposed to falls of 15.5 feet from Sky Jack SJIII 3219 scissors lifts in that the workers stood on platforms that were too narrow (midrails of the scissor lift guardrails), while welding bar joists and bridging. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement: (a) On or about April 4, 2018, employees were exposed to impalement hazards on uncapped rebar at the northeast corner of the concrete slab of the construction project. Welders were working above the rebar when welding the bar joists and bridging.
