29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:     a)  First Floor, Northwest Corner - On or about March 3, 2018, a mason installing concrete block from the platform of a tubular welded frame supported scaffold was exposed to a 17 foot 10 inch fall for the scaffold board he was standing upon extended 42 inches beyond its nearest horizontal support.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:   a)  First Floor, Northwest Corner - On or about March 3, 2018, a mason installing concrete block from the 2 foot wide platform of a tubular welded frame supported scaffold was exposed to a 17 foot  10 inch fall for the platform was not been provided with fall protection.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall:   a)  First Floor, Northwest Corner - On or about March 3, 2018, no falling object protection was provided in order to prevent buckets, tools, blocks and mortar pans from falling 11 feet upon workers below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:     a)  First Floor, Northwest Corner - On or about March 3, 2018, masons installing concrete blocks from the platforms of a tubular welded frame supported scaffold were not provided with a ladder or similar safe means of access and were exposed to a 11 foot fall from the scaffold's top platform.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
