29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment to be made by a competent person(s) designated by the employers in the following instances:   a)  Powers Built Structures, Inc. at 1200 Hidden Valley Parkway, Severance, CO 80550:  On or before April 4, 2018, the employer did not ensure that competent person inspections were performed on the steel erection of the west staircase in Area E.  Erection aid tabs used to connect the west and north horizontal tube steel beams to the columns were not designed, rated, and approved by a qualified engineer.  In addition, these erection aid tabs were tack welded to the west columns, and this welding was not inspected by a qualified weld inspector.  The employer did not ensure that competent person inspections were performed of the steel erection process of the west staircase of Area E;  b)  Powers Built Structures, Inc. Sampson Construction Co., Inc. at 1200 Hidden Valley Parkway, Severance, CO 80550:  At approximately 7:10 am on April 4, 2018, while setting up to pump concrete, a concrete pump truck boom struck the upper horizontal tube steel beam on the west side of the staircase in Area E.  The employer did not ensure that a competent person inspected the beam and its supports after this first strike, and before continuing with concrete pumping operations.  At approximately 10:10 am, a concrete pump truck struck the west upper horizontal tube steel beam with its boom a second time.  The beam fell and struck an employee of the concrete delivery company, resulting in fatal injuries.   These conditions exposed subcontractor employees to the hazard of being struck-by falling steel beams.

Erection aid tabs used to connect the west and north horizontal tube steel beams to the west columns were not designed, rated, and approved by a qualified engineer.  In addition, these erection aid tabs were tack welded to the west columns, and this welding was not inspected by a qualified weld inspector.  The employer did not ensure that structural stability was maintained at all times during the steel erection process.  This condition exposed employees, and employees of other contractors, to the hazard of being struck-by falling steel.
