29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a) On or about April 10, 2017, in the storage equipment trailer, the employer did not inspect portable flexible cords prior to start of shift for external defects such as but not limited to separation of the outer insulation from the plug.  Employees were exposed to electrical hazards.

29 CFR 1917.30(a)(3): The employer did not establish an employee alarm system that provided warning for necessary emergency action and for reaction time for safe escape of employees from the workplace or the immediate work area.  a) On or about April 10, 2018, the employer did not ensure that an employee alarm system was in place in order to warn employees so to allow for necessary emergency action and reaction time for safe escape of employees.  Employees were exposed to weather emergencies such as but not limited to weather and natural disasters.
