29 CFR 1926.453(b)(2)(iv): Employee working in an aerial lift were not standing firmly on the floor of the basket: a) At 1590 Eastern Boulevard, Montgomery, Alabama: On or about April 10, 2018 and at times prior; the employer failed to ensure an employee operating an aerial lift had his feet on the floor of the basket and not climbing the rails of the basket exposing the employee to a fall hazard of 26 feet to the pavement below.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee working from an aerial lift: a) At 1590 Eastern Boulevard, Montgomery, Alabama: On or about April 10, 2018 and at times prior; the employer failed to ensure an employee operating an aerial lift was wearing a harness and tied off exposing the employee to a fall hazard of 26 feet to the pavement below.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information: a) At 1590 Eastern Boulevard, Montgomery, Alabama: On or about April 10, 2018and at times prior; the employer exposed employees to chemical hazards to include but not limited to acrylic latex and base in that employees were not provided the new information and training on the UN Global Harmonized System (GHS) to include Safety Data Sheets (SDS).
