29 CFR 1926.451(a)(6): Scaffolds were not loaded in accordance with the design by a qualified person:  Or in the alternative   Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:  (a)  On or about April 10th, 2018, for the employees who were exposed to a fall hazard of up to 13 feet while installing house wrap from a 16 foot wide man basket that was attached to a JLG telehandler, Model 644E-42, in the vicinity of 3108 12th Avenue NW in Mandan, ND.
