29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  On or about April 10, 2018 on the pre-engineered metal building, employees were exposed to fall hazards of more than 15 feet while installing the decking on the roof without a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   On or about April 10, 2018 on the pre-engineered metal building, employees were exposed to fall hazards while using an extension ladder to access a roof whose side rails did not extend at least three feet above the upper landing.
