1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) New medical office construction located at 12250 Crabapple Road, Upper roof: On or about 04/09/2018, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic nail guns at 120 PSI, in that protective eyewear was not provided while employees were installing plywood sheathing.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose:    (a) New medical office construction located at 12250 Crabapple Road, Inside building near entrance: On or about 04/09/2018, and at times prior, the employer exposed employees to electrical shock/burn hazards while using a makeshift power strip, in that a metal junction/job box was used as a power strip which was connected to an extension cord. The electrical box is intended to be fixed to support within a structure.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    (a) New medical office construction located at 12250 Crabapple Road, Inside near front entrance: On or about 04/09/2018, and at times prior, the employer exposed employees to electrical shock and/or burns from electrical cords, in that the strain relief was damaged on two extension cords being used to power circular saws and reciprocating saws.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     (a) New medical office construction located at 12250 Crabapple Road, Upper roof: On or about 04/09/2018, and at times prior, the employer exposed employees to fall hazards of 6-feet, 6-inches to the lower level, in that a fall protection system was not provided to employees installing plywood sheathing on a 4/12 pitched roof.     (b) New medical office construction located at 12250 Crabapple Road, Lower roof: On or about 04/09/2018, and at times prior, the employer exposed employees to fall hazards of 13-feet, 6-inches to the lower level, in that a fall protection system was not provided to employees removing plywood from the forklift truck.
