29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Construction Site: On or about April 10, 2018, employees were observed working on a residential construction without the use of proper fall protection equipment. This condition exposed employees to fall hazards greater than 6-feet to the ground below.

29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee.    Construction Site: On or about April 10, 2018 the employer did not retrain employees in fall protection systems when inadequacies in the affected employee's knowledge were indicated.  This condition exposed employees to a fall hazard.
