Live parts to which an employee could be exposed were not de-energized before the employee worked on or near them:  a) Plant - On or about  04/10/18 employees assigned to the installation of a coolant expansion tank moved the position of a conduit that powered the outdoor lights through a photocell. The circuit was not de-energized before the work started, and employees were exposed to 277V live parts.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.23(c)(4): Unsecured or unstabilized ladders were used on surfaces that were not stable and level:       a) Site - On or about 04/10/18 and at times prior thereto employees assigned to the installation of a cooling expansion tank placed the Husky 12 ft., fiberglass, (A-type) step  ladder on top of a metal plate resting loose on top of two cylindrical horizontal pipes at a 12 in. elevation from ground. The width of the plate barely exceeded the width of the ladder at its base. The ladder was not secured to prevent tilting of the top, or slipping of the legs off the loose plate.         In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
