29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a) Guerreros Gutters and Roofing located at 50 Rainbow Circle, Gypsum, CO 80637: On and preceding April, 10, 2018, the employer did not ensure that employees used appropriate eye protection when operating pneumatic nail guns.  Employees engaged in residential roofing did not use safety glasses while installing shingles using pneumatic nail guns.  This condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     (a) Guerreros Gutters and Roofing located at 50 Rainbow Circle, Gypsum, CO 80637:  On and preceding April 10, 2018, the employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower levels were protected from falling by a guardrail system, a safety net system, or a personal fall arrest system.  Employees were installing shingles onto the roof of a two-story residential home without the use of fall protection.  This condition exposed employees to a fall hazard of approximately 16 feet.
