29 CFR 1910.67(c)(2)(v): A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift: On or about 4/11/2018, at the above addressed jobsite, an employee installing bending from an aerial lift was exposed to a fall hazard of approximately 16ft due to lack of fall protection equipment. IN THE ALTERNATIVE: 29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about 4/11/2018, at the above addressed jobsite, an employee installing bending from an aerial lift was exposed to a fall hazard of approximately 16ft due to lack of fall protection equipment.
