29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about 04/12/18 at the Rochester Convention Center, Rochester, New York; employees were exposed to an unprotected fall of approximately 40 feet when they climbed out of the aerial lift onto the blue beams, which support the ceiling in the north and south halls, then climbed up the diagonal beams and onto the air handling duct to operate the controls that raised and lowered the eight chandeliers.    b) On or about 04/12/18 at the Rochester Convention Center, Rochester, New York; employees were exposed to an unprotected fall of approximately 30 feet when they climbed out of the aerial lift onto the blue beams, which support the ceiling in the north and south halls, and set lights along the beam.     NO ABATEMENT DOCUMENTATION REQUIRED
