29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    a) Wood Crating shop: On April 10, 2018 and at times prior thereto the employees operated the Powermatic table saw without a blade guard in place.  This exposed the employees to an amputation hazard.       Note) Certification and documentation are required for this item.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:    a) Wood Crating shop: On April 12, 2018 and at times prior thereto a single, 110 volt, electrical outlet was missing a faceplate.     b) Wood Crating shop: On April 12, 2018 and at times prior thereto the cover on a four plug, 110 volt, electrical outlet was not properly or fully installed.
