On or about April 12, 2018, at the homes front elevation, left side to the entryway, two (2) employees worked from a supported scaffold system that did not bear on base plates and mud sills and were exposed to an 11-foot, 5-inch fall hazard.  b. On or about April 12, 2018, at the homes front elevation, center of the entryway, one (1) employee worked from a supported scaffold system that did not bear on base plates and mud sills and was exposed to an 13-foot, 4-inch fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  a. On or about April 12, 2018, five (5) employees accessed three separate supported scaffold systems, by climbing on the crossbraces in the absence of a ladder and were exposed to caught-in and fall hazards ranging from 10 to 16-foot.

On or about April 12, 2018, at the homes front elevation, left side to the entryway, two (2) employees worked from a supported scaffold without the aid of fall protection systems and were exposed to an 11-foot, 5-inch fall hazard.  b. On or about April 12, 2018, at the homes front elevation, center to the entryway, one (1) employee worked from a supported scaffold without the aid of fall protection systems and was exposed to an 13-foot, 4-inch fall hazard.  c. On or about April 12, 2018, at the homes front elevation, right side to the entryway, two (2) employees worked from a supported scaffold without the aid of fall protection systems and were exposed to an 16-foot, 2-inch fall hazard.
