29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about April 12, 2018, at a new building construction site located at 1755 N. Highway 66 in Catoosa, OK, employees were allowed to work from the awning and other locations under construction without fall protection, exposing employees to the hazard of falls.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 12, 2018, at a new building construction site located at 1755 N. Highway 66 in Catoosa, OK an employee was observed using a ladder that did not extend at least 3 feet beyond the top of a parapet wall, exposing the employee to fall hazards due to ladder displacement.
