29 CFR 1910.134(a)(1): In the control of those occupational diseases caused by breathing air contaminated with harmful dusts, fogs, fumes, mists, gases, smokes, sprays, or vapors, the primary objective shall be to prevent atmospheric contamination. This shall be accomplished as far as feasible by accepted engineering control measures (for example, enclosure or confinement of the operation, general and local ventilation, and substitution of less toxic materials). When effective engineering controls are not feasible, or while they are being instituted, appropriate respirators shall be used pursuant to this section: a) On or about April 19, 2018, an Exterior Columns Line Welder exposed to 1.25 times the Iron Oxide Fume Permissible Exposure Limit was not required to wear respiratory protection.

Local exhaust or general ventilating systems shall be provided and arranged to keep the amount of toxic fumes, gases, or dusts below the maximum allowable concentration as specified in 1910.1000 of this part: a) On or about April 29, 2018, the ventilation in the Exterior Columns Line Area was not adequate to prevent an Exterior Columns Line Welder being exposed to 1.25 time the iron oxide fume Permissible Exposure Limit.

29 CFR 1910.252(c)(6)(ii): Indoors. Indoors, welding or cutting involving zinc-bearing base or filler metals coated with zinc-bearing materials shall be done in accordance with paragraph (c)(3) of this section: a) On or about April 19, 2018, the fan arrangement in the ventilation hood over the galvanized welding table did not draw the welding fume into the ventilation hood. Ventilation smoke testing showed the ventilation hood blew air towards the galvanized welding table and would blow welding fume towards the welding table edges and into the work area.

29 CFR 1910.1000(a)(2): Other substances -- 8-hour Time Weighted Averages. An employee's exposure to any substance in Table Z-1, the exposure limit of which is not preceded by a "C", shall not exceed the 8-hour Time Weighted Average given for that substance any 8-hour work shift of a 40-hour work week: a) On April 19, 2018, an Exterior Columns Line Welder was exposed to 125% of the 8-hour Time Weighted Average Iron Oxide Fume exposure limit. The 8-hour Time Weighted Average exposure limit for Iron Oxide Fume is 10 mg/M3. The Exterior Columns Line Welder was exposed to an Iron Oxide Fume Time Weighted Average of 12.5 mg/M3 over 538 minute time period.

29 CFR 1910.95(c)(1): The employer shall administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent. For purposes of the hearing conservation program, employee noise exposures shall be computed in accordance with appendix A and Table G-16a, and without regard to any attenuation provided by the use of personal protective equipment: a) The employer had not implemented a hearing conservation program. On or about April 19, 2018, an Exterior Columns Line Welder was exposed to a 306 minute noise dosage of 61.0%. A noise dosage of 61.0% is equivalent to an 8 hour Time Weighted Average of 86.4 dBA. An effective hearing conservation program includes, but is not limited to, the following: 1. Audiometric Testing. 2. Providing hearing protection for employee use. 3. Employee training on the Occupational Noise Exposure standard, the effects of noise to the employee and training on the purpose of audiometric testing. 4. Ensure the Occupational Noise Exposure standard is available to affected employees. 5. Recordkeeping of exposure monitoring and audiometric testing.

29 CFR 1910.179(j)(3): Periodic inspection. Complete inspections of the crane shall be performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section, depending upon its activity, severity of service, and environment, or as specifically indicated below. These inspections shall include the requirements of paragraph (j)(2) of this section and in addition, the following items. Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: (I) Deformed, cracked or corroded members. (ii) Loose bolts or rivets. (iii) Cracked or worn sheaves and drums. (iv) Worn, cracked or distorted parts such as pins, bearings, shafts, gears, rollers, locking and clamping devices. (v) Excessive wear on brake system parts, linings, pawls, and ratchets. (vi) Load, wind, and other indicators over their full range, for any significant inaccuracies. (vii) Gasoline, diesel, electric, or other powerplants for improper performance or noncompliance with applicable safety requirements. (viii) Excessive wear of chain drive sprockets and excessive chain stretch. (ix) (Reserved) (x) Electrical apparatus, for signs of pitting or any deterioration of controller contactors, limit switches and pushbutton stations: a) On or about April 17, 2018, the overhead cranes had not undergone a Periodic Inspection since November 16, 2016. Periodic inspections shall be conducted every 12 months or less.
