29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift:  a)  North Building - Employees were observed working from a JLG aerial lift and did not have their lanyards attached to the boom or basket, exposed to an approximate 22 foot fall hazard to the ground below, on or about April 13, 2018.  b)  North Building - Employees were observed working from a JLG aerial lift and did not have their lanyards attached to the boom or basket, exposed to an approximate 23 foot fall hazard to the ground below, on or about April 13, 2018.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.760(a)(1):  Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems:  a) North Building - An employee was observed conducting sheet metal operations on the metal deck of a building and was not protected from an approximate 28 foot fall hazard to the ground below, on or about April 13, 2018.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
