29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  305 Leominster-Shirley Rd, Lunenburg, MA  On April 17, 2018 and at times prior, employees used powered industrial trucks (PIT) with front-end attachments that were not reflected on the data plate from the manufacturer.  The locally-made bucket and the 96-inch long forks that were used, changed the center of gravity and maximum lifting capacity of the PIT from the 48-inch forks listed on the data plate.
