29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: Construction Site: On or about April 20, 2018; the employer did not provide a training program to ensure employees recognize the hazards of electrical shock and procedures to follow to minimize hazards. This condition exposed employees to a fall hazard of more than 6-feet to the ground below.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: Construction Site: On or about April 20, 2018; employees were permitted to work in proximity of an energized electrical power service drop (240/480 volts) while erecting a second story stud wall. This condition exposed the employees to an electrical shock hazard.
