29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:  a)    USNS Zeus, Colonna's Shipyard, Cable Drum Machinery Room: On or about 04/04/2018, employees used a 110-volts metal exhaust fan to supply air into the space. The cord was pulled away from the fan housing causing strain on the internal connectors, as a result employees were exposed to a possible shock hazard.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2):  a)    USNS Zeus, Cable Drum Machinery Room: On or about 04/04/2018, employees worked from a platform that was not adequately guarded, as a result exposed to a 10 feet fall hazard.   No abatement certification or documentation required.

29 CFR 1915.89(g)(1): Before servicing machinery, equipment, or a system that has a lockout/tags-plus system, the employer did not ensure that the authorized employee, or the primary authorized employee in a group lockout/tags-plus application, verifies that the machinery, equipment, or system is de-energized and all energy sources isolated:  a)    USNS Zeus: On or about 04/04/2018, the employer did not ensure that an authorized employee verify all energy sources to the Cable Handling Drum were isolated, as a result employees were potentially exposed to possible hazards.   No abatement certification or documentation required.
