29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millenium Edition, December 2000, FHWA: a) Dutch's Masonry, at 1299 N. Knox Ct., Denver, CO 80204: On and before April 16, 2018, the exposing employer did not ensure that traffic control signs or devices used for protection of construction workers conformed to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millenium Edition, December 2000, FHWA. The employer did not provide adequate advanced warning signs to protect employees who were flagging and performing work on the street. This condition exposed employees to the hazard of getting struck by oncoming traffic.

Knox Ct., Denver, CO 80204: On and before April 16, 2018, the exposing employer did not ensure that barricades for protection of employees did conformed to the portions of the American National Standards Institute D6.1-1971, Manual on Uniform Traffic Control Devices for Streets and Highways, relating to barricades. The employer did not to provide adequate barricades to protect employees who were flagging and working in the street. This condition exposed employees to the hazards of getting struck by oncoming traffic.

Knox Ct., Denver, CO 80204: On and before April 16, 2018, the exposing employer did not ensure that modifications or additions which affect the capacity or safe operation of the equipment were made with the manufacturer's written approval. The employer allowed the use of a rough-terrain forklift, Genie Model GTH 844, to lift a bundle of rebar with nylon slings attached to one tine. Lifting of a load in this manner affected the center of gravity, carrying capacity, and the security of the load. This condition resulted in employee exposure to being struck-by falling and swinging loads.

29 CFR 1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form: (a) Dutch's Masonry, at 1299 N. Knox Court, Denver, CO 80204: On or about April 16, 2018, the employer failed during calendar year 2016, to electronically submit information from their OSHA Form 300A or equivalent by December 15, 2017. The establishment employed greater than 20 employees and is classified the construction NAICS code during calendar year 2017.
