29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. On or about April 14, 2018, and in times prior thereto, workers were using a scaffold that was 29 inches wide to access elevations of up to 18 feet without attaching the required outriggers. This exposed the drywall workers to a fall hazard of over 10 feet, located at the outlet mall, space #1045.

On or about April 14, 2018, and in times prior thereto, workers were using a Werner, narrow frame scaffold, to hang drywall, located at space #1045 at the indoor outlet mall. Workers were not trained to use the outriggers and to provide fall protection when stacking scaffold, exposing them to the hazards of falls.
