29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a. On April 25, 2018, on the roof, employees using pneumatic nail guns were not wearing safety glasses, exposing themselves to an eye hazard.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. On April 25, 2018, on the 5:12 pitch roof, employees performing roofing work were not protected from an 18 feet fall hazard by the use of a fall protection system.
