29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. Employees engaged in installing an outrigger were exposed to fall hazards in excess of 10 feet while standing on a wooden pallet being lifted in the air by a forklift without the manufacturers approval.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Employees engaged in installing an outrigger while standing on a pallet being lifted by a forklift were not trained on the requirements of fall hazards on scaffolding.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: Employees engaged in installing an outrigger were exposed to fall hazards in excess of 10 feet while standing on a wooden pallet without fall protection that was being lifted in the air by a forklift.
