(a) Site: At 850 South Barre Road, South Barre, MA 01074, at the rail landing at the Blue Warehouse, on 4/26/2018 the portable dockboard used to move materials from a railcar to the warehouse had a broken anchor and could not be secured to prevent the dockboard from moving out of a safe position.
