29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  a) Bush Wall Systems, LLC / 6935 Peach St., Erie, PA: On or about April 26, 2018 employees were working from a mobile scaffold greater than 10-feet from ground level. Guardrail systems were not installed at all open sides on the working level of the scaffold.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:     a) Bush Wall Systems, LLC / 6935 Peach St., Erie, PA: On or about April 26, 2018 employees were using A-frame ladders in the folded position and leaned against scaffolding to access both levels of scaffolding that was being used. A-frame ladders are intended to be used with the spreaders extended.
