29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 04/26/2018, at the construction site located at 1720 Clark Ave., Saint Louis, Missouri, the worker engaged in the instillation of a water-proofing system on the vertical concrete wall was not protected from falling into an excavation that was approximately 12 feet deep. The worker was approaching the edge of the excavation without any means of fall protection. The JAMES G. STAAT TUCKPOINTING & WEATHERPROOFING, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1138119, citation number 1, item number 4 and was affirmed as a final order on 05/09/2016, with respect to a workplace located at 1301 Main Street, Trenton, Missouri 63123.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about 04/26/2018, at the construction site located at 1720 Clark Ave., Saint Louis, Missouri, the portable ladder used to access an excavation that was approximately 12 feet deep was not extended three feet above the landing.
