29 CFR 1926.404(b)(1)(iii): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: a. a. Bethalview Shopping Center, 5740 Steeple Chase Road, Cumming, GA - Roofing subcontractors employees were exposed to electrical hazard. The GFCI receptacle which the roofing subcontractor was using to plug-in the portable circular saw for cutting of roofing material, was tested to be inoperable, on or about 04/27/18.

29 CFR 1926.701(b): Protruding reinforcing steel, onto and into which employees could fall was not guarded to eliminate the hazard of impalement: a. Bethalview Shopping Center, 5740 Steeple Chase Road, Cumming, GA - Roofing subcontractors employees were exposed to impalement hazard. Subcontractor employees were accessing the fixed wall ladder going to and from the roof, the unguarded steel rebar protruding 18" was in close proximity to where the employees were accessing the roof. The protruding rebar did not have a cover/guard on the end, to prevent impalement hazard, on or about 04/27/18.
