The most recent occurrence of this was documented on April 27th, 2018 on the job site located at the Southwest Corner of 54th Street and Newport Ave in Omaha, NE where the employer has employees operating a Genie model GTH1056 rough terrain forklift to move a 56 foot long Hydro-Mobile Scaffold platform.

The most recent occurrence of this was documented on April 27th, 2018 on the job site located at the Southwest Corner of 54th Street and Newport Ave in Omaha, NE where the employer has employees operating a Lull model 844-C Highlander rough terrain forklift to move a 56 foot long Hydro-Mobile Scaffold platform.

29 CFR 1926.602(c)(1)(ii):  Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval:   The employer is failing to protect employees from struck by and caught between hazards associated with modifying the lifting methods of a rough terrain forklift without consulting and having received approval from the manufacturer.  The most recent occurrence of this was documented on April 27th, 2018 on the job site located at the Southwest Corner of 54th Street and Newport Ave in Omaha, NE where the employer has employees operating a Lull model 844-C rough terrain forklift without a load chart in unison with a Genie model GTH1056 to move a 56 foot long Hydro-Mobile Scaffold platform, each lift was using a below the forks sling technique that is not covered in the operators manual and thus has no load chart as means of defining its capacity.
