29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:    A) Career Staffing, LLC, employing workers at 111 Weires Drive, Archbold, OH 43502: On or about March 27, 2018, and at times prior thereto, the employer did not perform authorized employee training, or ensure that authorized employee training was performed for temporary workers prior to allowing them to engage in servicing activities on the equipment.  Employees were not trained to isolate hazardous energy sources prior to engaging in servicing activities such as, but not limited to, changing mandrels/bars while setting up the left mandrel winder located in the Spring Department. As a result of this failure, employees were exposed to amputation hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    A) Career Staffing, LLC, employing workers at 111 Weires Drive, Archbold, OH 43502: On or about March 27, 2018, and at times prior thereto, the employer did not ensure that employees were protected from caught-in and amputation hazards. Employees were working in immediate proximity to the unguarded ingoing nip point created between the mandrel/bar and the wire and to moving parts such as, but not limited to, the rotating mandrels/bars and collars with two protruding bolts on the left mandrel winder located in the Spring Department. There were no safeguarding devices that would prevent employees from coming into contact with moving parts of the left mandrel and from exposing employees to caught-in and amputation hazards.
