29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) Jobsite, 799 West Chestnut Street, Washington, PA:  On or about May 2, 2018,  two employees were working on a ladder jack scaffold approximately 18 feet above the ground and were not utilizing fall protection.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  a) Jobsite, 799 West Chestnut Street, Washington, PA:  On or about May 2, 2018,  two employees were working on a ladder jack scaffold approximately 18 feet above the ground, that did not have a ladder available for access, therefore the employees used the attached scaffolding, which didn't have a ladder, and climbed down the crossbraces to a portable ladder leaning against the bottom.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a) Jobsite, 799 West Chestnut Street, Washington, PA:  On or about May 2, 2018, four employees were performing framing activities on a roof at heights up to 33 feet above the ground, without utilizing fall protection.
