29 CFR 1910.269(m)(3)(ii): The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be denenergized were open.  The employer did not render such means inoperable, unless its design does not so permit, and then sure that such means are tagged to indicate that employees are at work:  On or about May 2, 2018, employees performing repair work on an overhead power line were exposed to electrical hazards  when the employer did not ensure that all switches were open.

29 CFR 1910.269(n)(2): For any employee to work transmission and distribution lines or equipment as deenergized, the employer did not ensure that the lines or equipment were deenergized under the provisions of paragraph (m) of this section and did not ensure proper grounding of the lines or equipment as specified in paragraphs (n)(3) through (n)(8) of this section:  On or about May 2, 2018, employees performing repair work on an overhead power line were exposed to electrical hazards when the employer did not ensure proper grounding of the lines.
