Lower level employees were not utilizing head protection while working beneath the upper level employees who is handling portable hand tools and other roofing material, on or about May 3, 2018.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. 1596 Wilson Manor Circle, Lawrenceville, GA (existing residential home) - Employee were exposed to fall hazards of approximately 25 feet, while performing roofing work such as, installing roof shingles on the elevated roof area of a new residential structure without the use of fall protection, on or about May 03, 2108.

29 CFR 1926.553(a)(1): Exposed moving parts such as gears, projecting screws, setscrews, chain, cables, chain sprockets, and reciprocating or rotating parts, which constitute a hazard, shall be guarded:   a.   1596 Wilson Manor Circle, Lawrenceville, GA (existing residential home) - Employees were exposed to caugh-between hazard. Employees were using a TranzSporter platform hoist ladder to haul roofing material to the employees working on the roof.  The platform hoist was operating without having a guard to cover the running belt and pulley at the motor drum hoist, on or about May 3, 2018.
