29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: A. Guardrail systems; B. Safety net systems or C. Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: Epsom Building, Second Floor, Crystalizer Area. On or about May 2, 2018 employees were exposed to falls greater than 4 feet to the first floor below. The guardrails surrounding the floor holes around the crystalizer machine were not continuous. Employees used the lower rungs of a step ladder near the opening in the guardrails to reach the top of a filter for filter changes. There is no abatement documentation or certification required for this item.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables may not be used where run through holes in walls, ceilings or floors: Epsom Building, Second Floor, Control Room. On or about May 2, 2018, a flexible cord was run through holes in the walls and ceiling in order to provide power to a refrigerator in the Control Room, exposing employees to electric shock and fire hazards. There is no abatement documentation or certification required for this item.

29 CFR 1910.212(a)(4): Revolving drum(s), barrel(s), or container(s) were not guarded by enclosure(s) which were interlocked with the drive mechanism so that the barrel(s), drum(s), or container(s) could not revolve unless the enclosure(s) were in place: Metso Building, Revolving Kilns. The revolving anhydrous and pentahydrate kilns were not equipped with interlocked enclosure guards (or equivalent protection), exposing employees to the hazards of contacting the rotating drums. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
