29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.      a.) Clarksburg City Park: On or about 5-3-18, employees were more than 2 feet below the roof level and used the guardrail of the United Rentals Scissors lift to access the roof of the building.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a.)  Clarksburg City Park:  On or about 05/03/18, employees installing a metal roof on a 4/12 slope roof were not protected from falling 28'-36' to the ground below.    Trent Thrasher Construction, LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 954935, citation number 01 item number 001 and was affirmed as a final order on 03/02/14, with respect to a workplace located at 2493 Highway 77, Southside, AL 35907.
