On or about May 7, 2018, employees were exposed to a fall of approximately 15 feet while installing flashing materials on a residential home located at 783 Mentor, Akron, Ohio 44303. The employer did not ensure employees were working from a supported scaffold that was inspected by a competent person.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. On or about May 7, 2018, employees were exposed to a fall of approximately 15 feet while installing flashing materials on a residential home located at 783 Mentor, Akron, Ohio 44303. The employer did not ensure employees had fall protection while working from a supported scaffold.

Scaffold components manufactured by different manufacturers shall not be modified in order to intermix them unless a competent person determines the resulting scaffold is structurally sound. On or about May 7, 2018, employees were exposed to a fall of approximately 15 feet while installing flashing materials on a residential home located at 783 Mentor, Akron, Ohio 44303. The employer permitted employees to work from a supported scaffold constructed with a ladder made by a different manufacturer than the supported scaffold components.
