29 CFR 1910.23(c)(11): (11) Portable ladders used to gain access to an upper landing surface did not have side rails that extend at least 3 feet (0.9 m) above the upper landing surface: a) 17 Premium Outlet Blvd., Lee, MA: On or about May 4, 2018 an employee was observed using a portable fiberglass extension ladder to reach his roof top work location whose side rails did not extend any distance past the upper landing surface.
