29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: 77 Lefferts Ave on or about: May 7, 2018 a) Employees working on a roof exposed to a fall of approximately 10 feet. Employees removing roof materials and standing at the edge throwing materials into a dumpster below were not using any fall protection.

The ladders were placed on an angle and not level.
