29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) Site front roof: On or about 05/03/2018 the employer exposed two of his employees to fall hazards of 11 feet to the ground below by not ensuring fall protection was being worn while the employees were performing siding on a residential roof.

(a) Residential Site left side of roof: On or about 05/03/2018 employees were exposed to fall hazards of 11 feet in that the employer did not ensure employees performing siding work from an 8 in 12 pitch roof were certified in fall protection.

29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked or withdrawn from service until repaired.    (a) Site roof:  On or about 05/03/2018 employees were exposed to trip hazards in that the employer did not inspect the Werner extension ladder and remove it from service when it had a broken rung.
