29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501:    (a) ALM Roofing Services LLC at 2080 California Street in Denver, CO 80205: On or before May 8th, 2018, the employer did not insure that employees were protected from fall hazards while roofing a residential apartment complex.  Employees were installing TPO and flashing at the edge of a flat roof approximately 13 stories above the ground without the use of fall protection.  This condition exposed employees to fall hazards of approximately 130 feet.
