29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about May 10, 2018, and at times prior thereto, at the facility where an employee was exposed to a fall hazard exceeding six feet when working on top of chemical reactors, without a fall protection system in place.

29 CFR 1926.1053(b)(17): Fixed ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, or corroded components, were not withdrawn from service until repaired:  On or about May 10, 2018, and at times prior thereto, at the facility where employees were exposed to a fall hazard while using an fixed ladder to access the spin dryer work platform without having the bottom of the ladder secured to prevent the ladder from kicking in when being used.
