29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:       a) Job site, 6451 Stuebenville Pike Road, Pittsburgh, PA: On or about May 8, 2018, during HVAC and other construction activities, the employer did not ensure that an employee landing a load onto the roof was using appropriate head protection while working near items above his head that could have come down and struck him.

29 CFR 1926.1425(e)(1): Employees were not prohibited from being under the load:    a) Job site, 6451 Stuebenville Pike Road, Pittsburgh, PA: On or about May 8, 2018, during HVAC and other construction activities, the employer did not ensure that an employee was clear of a load being lowered onto the roof in that while the load was approximately 2 feet above his head, he reached up and grabbed the side of the load, and in the process was exposed to a crushing hazard had the load dropped.

29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:      a) Job site, 6451 Stuebenville Pike Road, Pittsburgh, PA: On or about May 8, 2018, during HVAC and other construction activities, the employer did not ensure that appropriate perimeter wall structures were of adequate height to serve as a guardrail system in that parapet wall heights ranged from approximately 18 inches tall to 29 inches tall, exposing employees to falls of up to approximately 16 feet.
