29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a. On or about May 2, 2018, located in the production area, employees were exposed to caught-between hazards of a #15 Floor-type Famco Manual Kick Press while performing setup and adjustment activities to the press. The employer failed to implement energy control application steps as the weight from a counter-balance was not blocked or otherwise  secured [1910.147(d)(3)] to protect the employees from the unguarded pinch points located at the stop collets. As a result, the remaining applicable energy control element involving verification of isolation [(d)(6), was not implemented to protect employees from the equipment hazards.   On or about May 2, 2018, located in the production area, an employee was making an adjustment on a #15 Floor-type Famco Manual Kick Press when the employee inadvertently cycled the press while the employee's hand was on the upper portion of the press where there were unguarded pinch points at its upper and lower threaded rod collets resulting finger tip crushing injuries.
