29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    On or about May 10, 2018, at the above addressed jobsite, an employee re-siding a house using a ladder jack scaffold was not provided training on how to minimize fall hazards when working from a ladder jack scaffold, thereby exposing the employee to the hazards associated with falls.

On or about May 10, 2018, at the above addressed jobsite, an employee performing siding installation operations on ladder jack scaffold approximately eighteen (18) feet above the ground was not protected from falls, thereby exposing the employee to fall hazards.
