29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  At the residential construction site located at 1721 SW 24th St, Blue Springs, Missouri:  Employees were working from a work platform that was placed on the forks of a Lull all terrain forklift.  Employees were exposed to a fall hazard in that the basket was not meant to be used by that powered industrial truck in the manner it was being used.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee was using eye protection when exposed to eye or face hazards from flying particles.  At the residential construction site located at 1721 SW 24th St, Blue Springs, Missouri:  Employees were exposed to struck by hazards when using an electric powered pneumatic nail gun without the use of eye protection.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  At the residential construction site located at 1721 SW 24th Street, Blue Springs, Missouri:  Employees were exposed to fall hazards.  Employees were not wearing fall protection while working in a man basket attached to a Lull all terrain powered industrial truck.  Employees were working approximately 27 feet in the air.
