29 CFR 1926.453(b)(2)(v): A body belt or harness  with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    Worksite located at 8900 NE Flintlock Road, Kansas City, Missouri- Employee was exposed to a fall hazard while working in an aerial lift. The employee had put on a full-body harness with a lanyard, but failed to put on the leg straps of the harness. If the employee were to need fall protection, the harness would not protect the employee from falling to the ground level.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     Worksite located at 8900 NE Flintlock Road, Kansas City, Missouri- Employee was observed working and walking on top of a built-up roof near unprotected edges and was not protected from falling by the use of any means of fall protection.
