29 CFR 1910.106(b)(2)(vii)(a): Area(s) surrounding tank(s) containing flammable or combustible liquids were not provided with adequate drainage or dikes:    (a) At the fueling area on the west side of the structure, on or about May 10, 2018 and at times prior to, two diesel fuel tanks exposed to heavy equipment traffic were not provided with adequate drainage or dikes, exposing employees to chemical and fire hazards. 29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (a) Throughout the facility, on or about December 6, 2016 and at times prior to, the employer did not provide basic advisory information contained in Appendix D to employees who used filtering face piece respirators on a voluntary basis, resulting in a respiratory and dermal hazard.
